Aftercare Program
Finishing an intensive phase of treatment is a milestone, not the end of support. Aftercare helps protect progress while routines, relationships, and confidence continue to grow.
Who this is for
Care matched to the need.
Aftercare may support people completing IOP, outpatient, residential, or another structured treatment program who would benefit from continued accountability and connection.
The frequency and length of aftercare depend on clinical need, prior treatment, goals, and any external requirements.
What to expect
A plan you can understand.
Specific schedules, eligibility, and recommendations are confirmed during intake and assessment. Your plan can change as your needs change.

A transition plan
Before stepping down, you identify risks, supports, warning signs, and next actions.

Ongoing check-ins
Scheduled contact keeps recovery goals visible and makes it easier to respond early to setbacks.

Relapse-prevention practice
You continue strengthening skills for cravings, stress, relationships, and high-risk situations.

Community connection
Peer, meeting, housing, employment, and other supports are added as appropriate.
Cost and coverage: Ask intake about Apple Health, insurance verification, and the sliding fee scale before your first visit.
