Case management supports eligible clients whose health and recovery goals depend on coordination across providers, agencies, benefits, housing, employment, or legal systems.

A case manager does not replace an attorney, benefits specialist, or housing agency. They help you understand options, make referrals, prepare for appointments, and follow through.

Specific schedules, eligibility, and recommendations are confirmed during intake and assessment. Your plan can change as your needs change.

  1. A needs assessment

    Together, you identify the barriers most likely to interrupt care or recovery.

  2. A prioritized plan

    The next steps are broken into manageable tasks with clear ownership.

  3. Warm referrals

    When possible, we connect you directly with trusted community and public resources.

  4. Follow-through

    Progress is reviewed and the plan is adjusted as circumstances change.
