Case management supports eligible clients whose health and recovery goals depend on coordination across providers, agencies, benefits, housing, employment, or legal systems.
A case manager does not replace an attorney, benefits specialist, or housing agency. They help you understand options, make referrals, prepare for appointments, and follow through.
